Loose Skin After GLP-1 Weight Loss: What One Non-Surgical Session a Year Can and Can't Do

Short answer: After weight loss with semaglutide or tirzepatide, mild to moderate laxity of the face, neck, upper arms, abdomen or inner thighs can be improved without surgery using Volnewmer (monopolar radiofrequency), Ultraformer MPT (focused ultrasound) or both in one sitting (VOLFORMER). Each is a single session, typically repeated once a year. Hanging folds of skin are different: no device removes excess skin, and that is a job for a plastic surgeon. Our practice in Kırklareli, Türkiye is about 2–2.5 hours from Istanbul Airport and about 30 km from the Bulgarian (EU) border.

Why skin loosens after GLP-1 weight loss

In the large trials, people lost on average about 15% of their body weight in 68 weeks on semaglutide 2.4 mg (STEP 1) and up to about 21% in 72 weeks on tirzepatide (SURMOUNT-1). Skin that stretched over years gets months to adapt. In a US survey of about 500 users, 38.7% reported skin changes — mostly facial volume loss, sagging and jowling — more often over age 55 and after losing more than 9 kg (Olivero et al., 2026).

On the face, imaging shows a median 9% loss of mid-face volume, roughly 7% per 10 kg lost, concentrated in the inner cheek, under-eye and temple fat pads (Sharma et al., 2025). On the body, the problem is mostly the skin itself.

Three grades of loose skin — and what fits each

  1. Crepey, thinner skin, no fold. The skin looks finer and less firm but doesn't hang. *Fits:* Volnewmer — radiofrequency heats the dermis evenly to firm its density.

  2. Visible sag, no overhang. The jawline softens, the upper arm "waves", the lower abdomen looks loose but doesn't fold. *Fits:* Volnewmer, Ultraformer MPT for the deeper support layers, or both (VOLFORMER).

  3. Overhang or fold. Skin hangs over itself — an abdominal apron, arm "wings", a fold under the jaw. *Fits:* surgery. We will tell you, and you can save the trip.

What each treatment actually does

  • Volnewmer: 6.78 MHz monopolar radiofrequency with water cooling; a 16 cm² body tip for the abdomen, arms and thighs, and a 4 cm² tip for the face and neck.

  • Ultraformer MPT: focused ultrasound delivered to fixed depths; on the face it reaches the SMAS at 4.5 mm. We use it for tightening — not as a weight-loss or fat-reduction tool.

  • Honest limit: most studies of these devices were done on age-related laxity. Evidence specifically in people who lost weight on GLP-1 drugs is still limited, so expect improvement, not a transformation.

When to schedule it

  • Weight stable for at least 3 months. Treating a body that is still shrinking means planning for a shape that will change. Even in surgery, a stable weight for at least 3 months before body contouring has been linked to fewer complications (see references).

  • If you plan to stop the medication, decide that with your prescriber first: in the STEP 1 extension, people regained about two-thirds of the lost weight within a year of stopping.

  • No need to pause your medication for these treatments: they are done awake, without sedation, so anaesthesia-related fasting advice for GLP-1 drugs doesn't apply. Don't change your dose on your own.

One trip, one session

Kırklareli is about 2–2.5 hours by car (≈190 km) from Istanbul Airport, around 45 minutes from Edirne, near the Greek and Bulgarian crossings, and about 30 km from the Bulgarian border at Dereköy–Malko Tarnovo.

  1. Before you travel: send photos of the areas (front and side, in daylight) on WhatsApp. If what we see looks like grade 3, we say so before you book a flight.

  2. Morning: examination and 3D skin analysis.

  3. Same day: treatment, with no downtime.

  4. Next day: fly home. Results build over 2–3 months; we review with photos at around three months.

  5. Next year: one maintenance session if needed.

Frequently asked questions

Can the abdomen and arms be treated in the same visit?

Often yes — it depends on the areas and the total treatment time. We plan this from your photos.

Will it help stretch marks?

Only modestly. Stretch marks are a different problem from laxity and usually need a separate plan.

How long do results last?

The new collagen stays, but ageing and weight changes continue. That's why maintenance is planned once a year.

Do I need someone to drive me?

No. There is no sedation; you can drive or travel the same day.
