Different types need different tools. A "universal acne cream" does not exist.
Step 2: The treatment ladder
Topical treatments. The foundation for mild to moderate acne: retinoids target the microcomedone — the invisible starting point of every lesion; benzoyl peroxide works against Cutibacterium acnes (the bacterium's current name) with the rare advantage of causing no bacterial resistance; azelaic and salicylic acid support unclogging and calming. Combinations matter more than single heroes — and combining a topical antibiotic with benzoyl peroxide rather than using it alone is how modern dermatology protects you from antibiotic resistance.
Systemic treatments. Oral antibiotics for a defined period in inflammatory acne — always time-limited, never open-ended. Hormonal therapy (combined contraceptives, anti-androgens) can be genuinely effective in women with a hormonal pattern, after proper evaluation.
Isotretinoin. For nodulocystic, scarring or stubborn acne, isotretinoin remains the most effective treatment we have — used under strict medical supervision with blood monitoring, and with absolute pregnancy prevention during treatment. Fear-based internet myths keep many patients from the one medication that could have prevented their scars; an honest consultation separates myth from monitoring.
Step 3: What about the scars?
Scar treatment is a separate discipline with its own logic — fractional lasers, RF microneedling and targeted techniques chosen by scar type. But the sequence is non-negotiable: active acne is controlled first, scars are treated second.Repairing old scars while new ones form is a circle, not a plan.
Honest notes on diet and skincare
The evidence suggests high-glycaemic diets can worsen acne in some people, and dairy shows a weaker, inconsistent link — reasonable to observe in your own skin, not grounds for extreme elimination diets. Skincare rules that actually matter: non-comedogenic products, no squeezing (it converts temporary spots into permanent marks), and sunscreen — because post-acne marks darken with sun.

Acne treatment is measured in weeks to months, not days: most treatments show real results at 8-12 weeks, and early quitting is the most common cause of "nothing works." Your plan is adjusted at follow-ups — treatment is a process, not a prescription.
