Telogen Effluvium (Diffuse Shedding): Why It Starts After Stress and When It Stops

Short answer: Telogen effluvium is a reactive, diffuse shedding that starts 2-3 months after a trigger and, once the trigger has passed, resolves on its own in most people within 3-6 months. It does not destroy follicles.


Typical triggers

  • High fever, COVID-19 or other infections

  • Surgery, anaesthesia, major blood loss

  • Childbirth (postpartum shedding)

  • Rapid weight loss, including GLP-1 medications such as semaglutide

  • Iron deficiency, thyroid disorders, some medications

  • Severe emotional stress


How it differs from pattern hair loss

Shedding is diffuse rather than patterned, hair comes out in handfuls when washing, and a gentle pull test is positive. Trichoscopy shows short regrowing hairs of uniform diameter. Because both conditions often coexist, a dermatologist looks for underlying androgenetic alopecia at the same visit.


What helps

Identify and correct the trigger: ferritin, thyroid function, vitamin D and B12 are checked. Supplements only help when a deficiency is proven. Topical minoxidil can shorten the recovery phase. PRP is optional support, not a cure.


When it is no longer simple

Shedding that continues beyond 6 months, a widening parting, or bald patches need re-evaluation for chronic telogen effluvium, androgenetic alopecia or alopecia areata.
