Botox for Chronic Migraine: Who, How, and With What Expectations

Botox is not only an aesthetic procedure — in chronic migraine it is a recognised medical treatment with its own protocol. But this is exactly where honesty is needed from the first sentence: Botox is not for every headache, and it is not the first option for every migraine.

Who is it indicated for?

​

The classic indication is chronic migraine: headache on 15 or more days per month, at least 8 of them with migraine features, for at least 3 months — usually in patients who have already tried preventive medication without sufficient benefit. In this group, studies show a meaningful reduction in headache days.

Who it is not the right answer for: infrequent episodic migraine, ordinary tension-type headache, and headaches that have not been medically assessed. Those have other treatments, and correct diagnosis precedes any injection. If you have not had a neurological evaluation so far, that step is discussed honestly at the consultation.

How it works

​

In migraine, Botox is administered according to a standardised protocol: superficial injections at defined points across the forehead, temples, back of the head, and the neck and shoulder region. The mechanism differs from the aesthetic use: the toxin blocks the release of mediators involved in pain transmission — it does not "relax wrinkles," it reduces the pain signal.

What expectations are realistic?

  • The effect builds gradually, over 2-4 weeks after treatment

  • The goal is reducing the frequency and intensity of attacks — not the guaranteed disappearance of every headache

  • Treatment is repeated at roughly 3-month intervals; in many patients the benefit increases after the second or third cycle

  • In some patients the response is modest — we tell you that beforehand, not afterwards

An honest bonus: because the forehead points partly overlap with aesthetic zones, many patients also notice a smoother forehead — a pleasant side effect, not the purpose of the treatment.

Safety

​

Side effects are usually mild and temporary: tenderness at the injection points, less often neck discomfort or temporary asymmetry. It is not used in pregnancy or breastfeeding, and neuromuscular conditions must be disclosed. The procedure takes 15-20 minutes with no downtime. Do not stop any of your medication on your own — Botox is added to your existing plan, not substituted for it.
