Patient Selection in Energy-Based Facial Rejuvenation: A Trainer's Framework

Short answer: In energy-based rejuvenation, the correct question is not "which device is best?" but "which layer is the problem in?" The framework Dr. Kağan Cingöz teaches in physician trainings rests on layers: SMAS descent → focused ultrasound; dermal laxity → monopolar radiofrequency; surface quality → fractional methods; volume loss → no energy device at all. Most faces need a planned combination.

The layer-based framework

Facial aging is not a single process: the deep support layer (SMAS) descends, the dermis loosens, surface quality declines and volume deflates — usually simultaneously. Treating one layer with one device leaves the others untouched; the patient concludes "the device failed," when in reality the device was aimed at the wrong layer.

  • 4.5 mm — SMAS: focused ultrasound (HIFU) reaches the layer surgeons tighten; the address for descended contours and jawline

  • Dermis: monopolar RF (Volnewmer, Thermage class) tightens the skin layer through volumetric heating

  • Surface: fractional CO2 and RF microneedling target scars, pores and texture

Honest exclusion: who should not get a device?

Thin faces with depleted fat pads risk fat loss at the wrong depth; advanced laxity exceeds what energy alone delivers, and the surgical conversation belongs before the procedure; unrealistic expectations are a contraindication in themselves.
