Facial Veins (Telangiectasia): Causes and Treatment

"Broken capillaries" — telangiectasias — are permanently dilated vessels, visible as fine red-purple lines on the cheeks, nose or chin. Two truths from the start: they do not clear on their own (a permanently dilated vessel does not shrink back with creams), but they respond very well to the right treatment — vascular-targeted light and lasers.

First: why did they appear?

Visible vessels are sometimes simply the result of sun, age and thin, fair skin. Other times they are a symptom — most often of rosacea — or a sign of factors still at work: sun exposure, extreme temperatures, alcohol, prolonged use of topical corticosteroids. The distinction matters enormously: treating the vessel without the cause is like emptying a bath with the tap running. If rosacea sits behind your vessels, the plan starts with controlling it — and the vessels are treated in the right order.

How the treatment works

Vascular-targeted light (BBL with vascular filters, dedicated vascular lasers) is absorbed by the haemoglobin inside the vessel: the energy converts to heat precisely in the capillary, which closes and is gradually reabsorbed by the body — without affecting the surrounding skin. Fine, superficial vessels are the most rewarding; thicker or bluish ones may need different wavelengths or more sessions.

What to expect realistically

• Treated vessels usually fade over a few weeks; many cases need 1-3 sessions, determined at assessment

• Immediately afterwards: redness and warmth for a few hours, sometimes a brief darkening of the vessel before it fades — part of the process, not a side effect

• Treated vessels do not come back — but the skin can make new ones if the factors (sun, uncontrolled rosacea) continue. Daily sun protection is the maintenance half of the treatment, not a luxury

• Anyone promising "every vessel gone forever in one session" is selling, not advising

How the session works

Examination (vessels, phototype, signs of rosacea) → the session: usually 15-30 minutes → soothing gel and instructions. No downtime for most patients; make-up and intense exercise wait for the recommendation. Tanned skin postpones the session — which is why autumn and winter are the classic vascular season.

Phototype: the honest note

Vascular light treatments are most effective on fairer skin, because in darker skin part of the energy is absorbed by melanin. That does not mean "impossible" — it means that with a darker phototype the choice of technology, the settings and the physician's experience become decisive, and sometimes the safer answer is a different method. That is assessed at examination.

When it is not performed

Pregnancy (postponed), recent tanning, active infection in the area, certain photosensitising medications (tell us everything you take) and unexamined skin — facial vessels are assessed, not "shot at with a laser" without an examination.
